Good Faith Estimate & Federal Transparency Notice

This practice is private pay and does not bill insurance directly.

In accordance with federal law, the following notice is provided regarding your rights as a client.

Private Pay & Superbills

Services are paid directly by the client at the time of session.

If you wish to seek reimbursement from your insurance provider, a superbill (a detailed receipt for services) can be provided upon request. Reimbursement is determined solely by your insurance plan, and this practice cannot guarantee coverage or payment.

Clients are encouraged to contact their insurance provider directly to understand their out-of-network benefits.

Your Right to a Good Faith Estimate

If you are uninsured or choose not to use your insurance benefits, you have the right to receive a Good Faith Estimate outlining the anticipated cost of your care.

Under federal law:

  • You may request a written Good Faith Estimate before scheduling services or at any time during treatment.

  • The estimate will include the expected cost of services based on the agreed-upon fee and anticipated frequency.

  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you have the right to dispute the bill.

Fees are discussed transparently prior to beginning treatment. There are no hidden fees or facility-based charges.
